- N2, Chapter 8 has no "Obtaining the Output" instructions section at the end. Rather, instructions are embedded in the chapters.

- p. 143 (important: when setting the categorical variable Stage to have the first category as reference, be sure to hit the "Change" button - clicking the "First" radio button is not enough);
- p. 146;
- p. 147 (again, remember to click the "Change" button);
- p. 149 (you may click the Reset button before entering the specifications but then re-enter thetime, status, and covariates as before. );
- Last paragraph of p. 150 (the dfBeta for age should be saved in your data sheet as DFB1_1, as created on p. 147; to get the y=0 reference line you have to go into the Chart Editor by right-clicking to select Edit Content, then right click the plot when in the Chart Editor);
- Second paragraph of p. 152 (the partial residual for age should be saved in your data sheet as PR1_1, as created on p. 147; to get the y=0 reference line you have to go into the Chart Editor by right-clicking to select Edit Content, then right click the plot when in the Chart Editor); also add a linear regression line in addition to the loess smoothing line, though Norusis doesn't. Right-click to select "Add fit line at total" under the Fit Line table of the Properties dialog of the Chart Editor window.

- p. 155. To get Fig. 8-16, rerun the model after deleting age as a covariate, then plot recomputed martingale residuals against age.
- pp. 156-157. Be careful to specify sex, age, and his as categorical with deviation contrasts with "last" as the reference category. The tables in Norusis are actually segments of output tables you will obtain.
